Dear J.P. Case Parents/Guardians:
The fall sports registration deadline is quickly approaching. Please be sure to sign your child up and turn in all the necessary paperwork so our school doctor can clear students; as tryouts are scheduled to begin in September.
The DEADLINE TO REGISTER IS Thursday August 7, 2025, for the following sports programs!
Varsity Cross Country (Coed)
Varsity Field Hockey
Varsity Girls Soccer
Varsity Boys Soccer
Varsity Girls Volleyball
IMPORTANT INFORMATION
The medical eligibility form completed by your child's physician, along with the Health History Questionnaire, must be dropped off at the JP Case main office by August 7th.
The main office is open Monday - Thursday, 8 am - 3 pm. You must also register your child in Family ID. Please double-check your work to avoid any unnecessary delays. No student-athlete can try out until all forms are submitted and cleared by our school physician. PLEASE DO NOT EMAIL ANY FORMS!
