Attention All RFIS Families
A Reminder from the Health Office

Asthma Treatment Plans, Allergy Action Plans, Daily/As Needed Medication Forms and all student medications are due Thursday, August 14, 2025.

Parents, please be sure to deliver all medication forms, printed in hard copies, along with your child’s medications in the original packaging (i.e. inhalers, Epi-Pen, etc.) to the RFIS Main Office by August 14. Office hours are Monday-Thursday, 7:30 AM-3:30 PM.

If there has been a change in your child’s health history or medications, please provide documentation from your medical provider.


Attention 6th Grade Families

Meningococcal and Tdap vaccines are required for 6th-graders when they turn 11 years old.  If your child’s birthday is after the start of school, please notify the school office of your child’s immunization appointment as soon as possible. Please provide documentation from your medical provider once vaccines are administered.

Students who have not met state requirements may be denied entrance to school in September 2025. 

Families requiring financial assistance should contact the Hunterdon County Childhood Vaccine Program at 908-806-4570.
