Last Reminder for Fall Sports Registration ~ Forms Due Tomorrow, August 7.
As sports registration comes to a close, please be reminded that in addition to being registered in Family ID, you also need to submit all paperwork for clearance to the school nurse (HHUQ/ Medical Eligibility forms). Please be sure to hand in hard copies of the HHUQ and Medical Eligibility form to the main office so the nurse can review all documents necessary to have your child cleared by our School Physician. Registering your child in Family ID is only step 1 of the 4-step process. The deadline is this Thursday, August 7.
Please be sure to sign your child up and turn in all the necessary paperwork so our school doctor can clear students; as tryouts are scheduled to begin in September. Fall Sports are as follows: Varsity Cross Country (Coed), Varsity Field Hockey, Varsity Girls Soccer, Varsity Boys Soccer, Varsity Girls Volleyball.
The medical eligibility form completed by your child's physician, along with the Health History Questionnaire, must be dropped off at the JP Case main office by August 7th. The main office is open Monday - Thursday, 8 am - 3 pm. You must also register your child in Family ID. Please double-check your work to avoid any unnecessary delays. No student-athlete can try out until all forms are submitted and cleared by our school physician. PLEASE DO NOT EMAIL ANY FORMS!

Two 90-minute delayed openings and two early dismissal days have been added to the calendar for students only. Staff will report for a normal day. The changes are as follows:
Monday, December 1 - 90-Minute Delayed Opening for students only
Monday, February 9 - 90-Minute Delayed Opening for students only
Monday, March 9 - Early Dismissal for students only
Wednesday, June 17 - Early Dismissal for students only.
